  • Individuals and organizations seeking a donation must submit requests via the online application found below.

Giving Guidelines

Please allow FOUR weeks upon receipt of request for a response via e-mail and/or letter. No phone calls, please. All request for donation forms must be completed in full to be considered. By submitting a request for donation form, you agree that any donation received is not to be used for resale. All donations received from B&W Pharmacy are for the intended use and the organization as noted.

Guidelines and Conditions: Since B&W Pharmacy may get several requests each day to process, please allow FOUR weeks upon receipt of request for a response via e-mail and/or letter. No phone calls, please. If donation request is approved, donation pick up times are: Monday through Friday 9am to 5 pm and Saturday 9am to 12pm. All request for donation forms must be completed in full to be considered. Attach any additional information regarding the event to this form you wish to be considered.
